Like its predecessor Kenzan, Ishin takes place in feudal Japan, this time during the 1800s and 龍が如く維新 focusing on the life of Ryoma Sakamoto, another real-life figure in Japan's history. After the murder of both his mentor and father figure, Sakamoto goes undercover to find out the identity of the swordsman who took them down.

They left a debt of 100 Ryo and a man says he is going to take the family home away to pay it off. Haruka is distressed by this as the home holds a lot of memories for her. A Shinto priest is pleading with the man to give her time to pay off the debt as she is only a child, but the man is unmoved. Ryoma says that he will help to look after her and help to pay off her debt, but she will have to work for it.

Substories and minigames once again returned, with the fan-favorite karaoke mini-game being included. There are also quirky little gameplay elements, such as Sakamoto being able to befriend animals who then retrieve items for him and a new side-game battle dungeon. The main adventure is completed with sidestories including minigames and a hundred "sub-stories". Like many games in the series, weapons and armour can be upgraded to new versions using items found in the world.
